Help with SuperMicro and Mellanox ConnectX-4-based NIC - no network interfaces are appearing?

FINAL UPDATE - The answer was the FEC mode! By forcing the switch port to Reed-Solomon, I was able to get it work!
Turns out the Mellanox only like RS-FEC - and the Arista's default to no FEC for 100Gbase-LR4. I asked Arista, and they said that was per the standard.
See UPDATE 3 below - The same optic/cable works fine in a EXFO FTB-1 - able to get a DHCP lease and ping gateway (https://i.imgur.com/aoF1e15.jpg). So seems to be Mellanox and this optic? I also added 7dB optical attenuator - no change unfortunately.
See UPDATE 2 below - I tested and it works with DAC cables - I am still trying to figure out why the Finisar optics aren't working? Any thoughts?
I have a SuperMicro server, and I've just installed a new SIOM (proprietary SuperMicro form factor) network card - AOC-MHIBE-m1CGM.
From the specifications, this seems to be based around the Mellanox ConnectX-4 chipset.
The SuperMicro website mentions it can do either Infiniband or 100Gbps Ethernet.
(I'm pasting screenshots, because the box doesn't have network connectivity yet, and I'm doing this over IPMI)
Anyhow, when I check ip addr listed, I don't see the 100Gbps interface come up (either on Infiniband or Ethernet):

If I check dmesg output, I do see "mlx" mentioned:

Any ideas what's going on? Is there something wrong with the card, or do need to do something special to get it to work?
UPDATE: OK - I was able to get the interface to show up by changing it from IB (Infiniband) to Ethernet - thank you Reddit community!
Also - the reason it didn't show up in lspci is PEBKAC - I was grepping for "mellanox", but it's "Mellanox", with a capital "M" =).
However, the interface is still not coming up.
For reference, this is how I changed the mode:

Any ideas what's going on, or why the Finisar 100Gbase-LR4 optic wasn't working?
UPDATE 3: I can confirm that the same Finisar optic and cable work fine in an EXFO FTB-1 network tester (https://i.imgur.com/aoF1e15.jpg). I was able to get a DHCP lease and ping the upstream gateway.
So at least from what I can tell - this is something about the Mellanox card and this optic/cable?
Also - I added a 7dB attenuator on the Rx side for both the Mellanox card and the Arista switch side.
Arista transceiver info with the attenutator in place - versus around 1 dBm Rx before:

World Championship resources

Format

  • Group Stage (October 1st-4th, October 8th-11th)
    • Double Round Robin Best of One matches (each team plays the others in its group on each side once.)
    • Tiebreakers will be held if necessary.
  • Knockout Stage
    • Top two teams from each group advance to the Quarterfinals.
    • Random draw after group stage with the following restrictions:
    • 1st place teams will face a 2nd place team
    • No two teams from the same group can be placed in the same half of the bracket
    • All Matches are Best of Five.
